Do you remember when HCFA, (a.k.a. CMS), released new documentation guidelines about ten years ago…. And then a few years later released “new†guidelines but promised to eventually release a totally revamped version… but you’ve been puzzled why that, frankly, never materialized? Well, it turns out the reason they’ve taken so long in “updating†those old guidelines was because they were so busy with developing new unfunded mandates for hospitals and physicians they simply didn’t have time. In an effort to consolidate these efforts, it seems the new HCAHPS surveys will prove to be the critical factor to drive future healthcare reimbursement. Although government officials refuse official comment, (it’s only official if official government officials release an official comment), this algorithm derived by Sharon Nelson, RN may just be the ultimate model of healthcare reimbursement the government has been looking for. As you can see, they are easy to understand. What a breath of fresh air from the typical bureaucratic/legal mumbo-jumbo that’s frustrated anyone every trying to get paid from Medicare. No longer will Medicare rules/regulations be obfuscated and inscrutable!
The federal government, in its ongoing effort to simplify and educate physicians and other healthcare dots who want to earn a living in healthcare, present this sample in an version that can be carried with you at all times for quick reference. Simply print out all these as pocket sized cards and glue them together, (back to back), to create a handy reference.

For ease of use, these instructions are provided AFTER * printing out cards)
1. Take card 1 and turn it face down. (Now you can’t see the writing).
Step 1. Take card 2 and glue the part of card 2 THAT ALSO HAS NO WRITING to Card A, (naturally, to the part of card A that also doesn’t have written information printed on the side that you’re actually gluing from Card 1, where card 1 is also the same as Card 1, because, again, obviously, these are not case sensitive cards.)
Step A. Once you have completed all of the above IN ITS ENTIRIRINGTY Proceed to step B.
Step C. Take the two cards which by now have been glued together, (allow sufficient time to sufficiently allow the glue to sufficiently dry… that is to say, the glue that you’ve used for ALL THE ABOVE STEPS), and simply turn them over, (collectively).
Step 1′ (prime) NOW, glue the final card (D) on two the too cards, to, (3) you’ve previously glued together on their back side, (the one without any printing). You have complet freedome to choose which one of the first two cards which have been glued together to use as the “back,” as this is a non-discriminatory methodology to either right handed or left handed preferred individuals.
Step 4. Your handy pockett reference is now ready for use.
